For Parents-Youth Prescription Drug Use Survey- Receive a FREE Med Lock Box

Attention parents!:

Please see below for a link to a survey for parents which is going to be utilized to assess parent perception as it relates to the issue of prescription drug and over-the-counter drug use.

The first 85 people who complete this survey are eligible to receive a free LockMed box for their homes in which they can store their medications safely. One LockMed device per household. The last question of this survey will ask for your email address. We will contact you via email to confirm you are eligible (while supplies last) to receive the LockMed box and provide additional information about pickup dates and time.

Any questions in this survey can be skipped. This survey and lock box incentive is sponsored by the Putnam County Communities That Care Coalition in partnership with the American Medical Association's Healthy Living Grant.
